Transportation Request Form
TRANSPORTATION REQUEST -BOOKING YOUR OWN FLIGHTS
This form MUST be submitted at least 30 Days prior to your travel dates!!!
*** DO NOT book flights until you have received a confirmation number from the resort***
- FREE ROUND TRIP TRANSPORTATION from airport POP (Gregorio Luperón International Airport) to resort and back is included
- Transportation from your home to the airport is NOT included.
- Your flight itinerary MUST be emailed to your Harmony Travel representative at least 30 days prior to your travel dates guarantee your transportation from POP (Gregorio Luperón International Airport) to LHVC Resorts
- Your transportation confirmation will be emailed to you once a confirmation is received from the resort.
- Harmony Travel/Karen’s Catering, LLC will arrange transportation for you when using the Flight Purchase Program. You will NOT need to submit a travel itinerary if you are using our Flight Purchase Program.
- You will be responsible to arrange your transportation from the resort back to POP (Gregorio Luperón International Airport) upon arrival at the resort
- It is your responsibility to submit your transportation request
FLIGHT INFORMATION MUST BE RECEIVED at least 30 DAYS prior to your travel dates to receive complimentary transportation to the resort!
